Payment & Non-Refundable Fee Policy

A Life in Balance DBT Skills Training Program

This confirms the terms of the enrollment and first payment you have completed for the DBT Skills Training Group. Please review the plan you enrolled in, read the policy below, and sign to confirm your agreement.

The Plan You Enrolled In

Payment Policy

Enrollment in the A Life In Balance® DBT Skills Training Group requires a financial commitment to the full 24-week program.

Your first payment—whether weekly, monthly, or paid in full—is collected at registration and reserves your seat in the group.

• Weekly Plan: First week paid at registration; weekly billing begins 7 days after the first session

• Monthly Plan: First month paid at registration; monthly billing begins 30 days after the first session

• Pay-In-Full Plan: Total fee paid at registration; no further charges

All clients must maintain an active payment method on file for recurring plans.

Non-Refundable Fee Policy

All DBT Skills Training Group payments are non-refundable, including:

• Enrollment payments

• Weekly or monthly recurring charges

• Pay-in-full fees

• Payments made prior to withdrawal or missed sessions

This applies regardless of your ability to complete the program.

Eligibility exception: If you are not approved as clinically eligible after the required intake and screening, any payments made are refunded in full.

If we cancel: If A Life In Balance cancels the program or is unable to deliver scheduled sessions, you will receive a prorated refund for any sessions not provided.

Why This Policy Exists

This policy maintains the integrity of the therapeutic group experience:

• Group spaces are extremely limited

• Your enrollment prevents another person from taking that seat

• Clinical preparation and administrative resources begin immediately after registration

• Group stability is essential for therapeutic success

Missed Sessions

Missed sessions are not refunded or prorated. When available, you may view the session recording for skills review, and handouts or summaries may be provided. Make-up sessions are not guaranteed and are offered at the facilitator’s discretion.

Acknowledgment and Agreement

By enrolling in the DBT Skills Training Group, you acknowledge and agree to:

• The full 24-week financial commitment

• The non-refundable nature of all payments

• The recurring billing schedule associated with your chosen plan

I have enrolled in and made the first payment for the DBT Skills Training Group under the plan I checked above. I have read, understand, and agree to this Payment & Non-Refundable Fee Policy, and I authorize the recurring billing schedule for my plan to continue on the payment method on file for the full 24-week cycle.

Insurance & Reimbursement (Superbills)

A Life In Balance is a private-pay practice, and we do not bill insurance directly.

If your plan includes out-of-network benefits, we can provide a monthly superbill (an itemized receipt for services) that you may submit to your insurance company for possible reimbursement.

How reimbursement typically works:

• You pay for services at the time of service.

• You request a superbill (provided on a monthly basis).

• You submit the superbill to your insurance company for reimbursement consideration under your out-of-network benefits.

Important notes:

Reimbursement is determined by your insurance plan and is not guaranteed.

• You are responsible for verifying your out-of-network benefits, coverage, and reimbursement rates directly with your insurer.

• A superbill reflects only billable clinical services and requires a qualifying diagnosis.

• On request, we can share the information insurers commonly ask for (service/procedure code, diagnosis code, and provider details) so you can confirm your benefits in advance.
